Advice for completing Health Assessment questionnaire? I have depression and anxiety

Hi everyone, I recently moved areas and had a reminder letter from the Health Assessment Advisory Service. I have to fill out a form, I was on ESA support group previous years. I would like some help to describe depression & anxiety as I feel this the only section I truly deserve any points my mobility is not a issue. This letter could mean the end of my basic UC so I'm mega stressed. Thanks guys in advance for any helpful replies

Firstly I would look at the descriptors and what points you feel you would get. And try think of examples.
For example coping with change, i struggle with change and when something changes, I become extermy anxious (I can come across as rude at times) simple things like someone comming early or late can trigger thisI work towards the time we said.. even tho rationally I know that people can't always be bang on time, my anxiety heightens. The other day I went to an appointment at 3 (had a text message with confirmation to prove it) but they had it down for 4. So the lady who I usually see, couldn't see me and had a full blown panic attack didn't do the appointment, went home and couldn't calm down for a few hours. So now I've said I'm not going there again as I do not want to feel like that.
Hope this kinda helps give you some ideas. But if you have a mental health worker or someone similar involved. They may be able to help you as they will no more about your circumstances and how anxiety and depression effect you.
