Will they accept this evidence and what name should I use?

It’s just occurred to me that when I requested the DLA forms I did it for my son in his birth name, which is the name on his child benefit and birth certificate. 

Our paediatrician, his OT and his school have written supporting evidence using his chosen name rather than this dead name.

Will they accept it as evidence as it’s a different name? And when I’m filling in the forms can I use his chosen name rather than the dead name when writing about him?

    Hi there 

    I would make it clear on his form you are referring to his chosen name and what his legal name is as well to avoid confusion 

    If you are sending the evidence with the form It should be OK 

    But if sending separate it may be confusing and may not get attached to his claim
