Hi @charl1980 - I have a permanently partially dislocated right collar bone, & like yourself am right handed, so just explained the difficulty I had in raising my right arm, & the pain I experienced. So, just as you have done here, you say the difficulty you have, & why. Try to give a couple of recent detailed replies for each activity that applies.....what happened, when, was your partner there, what did they have to do to help you, & why. Also write about why you can't manage a shower.

When it comes to evidence for pip I only havey experience, a letter from who I get councilling from and if they can contact my gp but I will go into great detail in form0

Don't rely on them contacting anyone because they rarely do this. The best evidence you can send is your own anecdotal evidence. You are the only person that knows exactly how your conditions affect you. Ideally you should put 2 examples per descriptor that applies and use one A4 sheet of paper per descriptor.

Examples been example of dates times or just how it affects me0

Detailed examples of what happened the last time you attempted that activity for each descriptor that applies. What exactly happened, where were you, who seen it etc.
