Spilled coffee all over PIP form, can I order another one or download the form?

I had to order a second PIP2 form as I lost the previous. My carer has filled it out but I spilt coffee all over it making ink blot. A ink pen was used rather than biro. Can I order another one or download a pip2 form. Can i send the form and evidence via email if time is an issue

It’s not available to download. You will need to ring PIP to ask them to send you another. Do be aware of the timescale to return the form although you can ask for an extension.0

In this thread here https://forum.scope.org.uk/discussion/comment/496643#Comment_496643 you said your carer photo copied the blank form. Why can't you print this out?
