Mandatory reconsideration letter

Hi,
Can anyone help with a link to an upto date mandatory reconsideration letter template please?
I rang dwp this morning to let them know that I STILL havent received my assersors report or the decision makers outcome letter, after it apparantly was posted to me weeks ago?🤔
I need this to then cross reference where I believe I should have scored more points etc.
This whole process has really affected my anxiety&depression, I feel like Im constantly having to fight my way through obstacles to get to the finish line of just being awarded what Im intilted to, its a horrible process.
I want to get my (MR) sent off in writing ASAP, the advisor told me to input any new letters I have from family/friends sharing how they see the way my disabilities affect me and to do my (MR) in writing. Does anyine have any advise on MR recently?

You can just write a letter rather than use a template.There’s also a form here but you will need to print it off before sending it. https://www.gov.uk/government/publications/challenge-a-decision-made-by-the-department-for-work-and-pensions-dwpIf you didn’t score any points then you can just go ahead and write the letter without waiting for the decision letter.0 -

Have you been informed of your award outcome? You mentioned not having yet received the assessors report or a decision letter.
There is a page over on the Citizens Advice website that provides information about challenging a PIP decision, and it has a section that includes a link to a template that you can use when putting together your MR. I hope you find it helpful.
I'm sorry to hear of how much the process has impacted your mental health. Can I ask, do you currently have any support in place to help you manage your mental health? For example, is your GP aware of your anxiety and depression and how they impact you?0
