What is the best way to go about getting a bus pass?

i have a severly disabled brother with autism and i have mild autism as well i was diagnosed at biminghams childrens hospital with my brother but for some reason it's not on the system records so my mum has inquired to see if they can find it on my records i really need a bus pass so i can start doing things i want. What would be the best way for me to go about it ?
