Pip assessment form is completely wrong

I got my pip assessment form in the post today and just started reading it. It’s as-if it’s about a different person. She’s put that I play piano, I’ve never played piano, wouldn’t have a clue where to start. Then that I can get dressed alone, my mum helps me daily, I can shower by myself, my mum washed my hair and helps me in and out, I can walk 100m unaided, I can walk 20m with crutches on a good day. 

What do I do about this?

    This is, unfortunately, something that many people find if they ask for a copy of the assessor's report. The assessor's report isn't the final decision, so I'd encourage you to wait until the decision letter comes through to see whether you've been awarded and, if so, the level of your award.

    If you don't get the award you believe you're eligible for, you can challenge the decision. You can read about that on the Citizens Advice website.

    I can imagine that reading things written about yourself that aren't true might be frustrating and upsetting. How are you feeling?

    Replied on a different thread. Deppi, multiple people report that the assessor hasn’t listened to them and have wrote false things on the report but this does not mean the report has been ‘mixed up’, these things happen sadly. Just wanted to let you know so you could bear that in mind.
    Another case of why requesting the report can cause huge anxiety.

    @Tori_Scope I feel fine, just very confused. I don’t feel anxious it’s just that there is obviously something wrong with the assessment. According to this I’ll get nothing. She’s not even wrote about half my medical issues and states I don’t see the gp about my depression but that’s the only person I see and I see her monthly, the same with my asthma. Some things I completely agree with but some aren’t me at all

    @deppi I’m currently on hold to dwp just to see what they recommend. Some areas I agree with but so much is missed out and completely incorrect. I’m not sure what’s gone on but this isn’t just a case of them misunderstanding.

    Dwp have put down on the system the issues and suggested I write a letter stating why I disagree with things. I think I might give it a few weeks to see what the actual report says then go from there. He said my assessment is currently being looked at so it should be around 6 weeks before I get an answer. Maybe I should go and learn to play piano 😂

    You have done the best thing and I would follow up with the letter as suggested then it's possible it will be looked at b4 making a decision 

    Keep a copy in case you need it for appeal and put your name and ni number on all pages

    This is the problem it causes when you request a copy of the report. Lots of people also think that the assessor should write down what you've said word for word, this isn't what should happen. The assessment process is there for them to gather more evidence to support your claim. The report is then written based on their opinion of everything you said in the form, your evidence and what you said during the assessment.
    Yes, the part about the piano playing isn't true but this doesn't mean the report belongs to someone else.

    @poppy123456 the piano isn’t the only thing that is completely wrong. The majority of the report doesn’t reflect me, most of my health conditions are missing, the ones that are there are spelt wrong and have incorrect information. My doctor asked me to request the form so she could look through it and even she says it’s like reading a form for a different person. They missed so much off and added so much that’s not true. I’m not stressed over it or anything, it was quite amusing to read especially when it was meant to be about myself, I learnt that I could apparently play piano too which was interesting. Maybe it’s a sign to start

    What may have happened is the notes that the HC had written down for someone else may have unintentionally got put in your report.

    They're were things in my report which felt like they had got it from someone elses assessment, the DM still awarded me.

    The decision lies totally with you how you want to go about it because it is your life which is affected by the decision of the DM.

    @chichi I wouldn’t normally bother with doing anything and just wait and see but I don’t think I’ll be awarded anything if they follow the assessment sadly.

    Have you actually looked at the dots next to the acitivities/descriptors to see what is recommended? If not then you can use the PIP self test by putting the dots from the report into the test. At the end it will add up any possible points recommended. https://www.benefitsandwork.co.uk/pip-test-form

    @poppy123456 she had added ticks on the last pages so I added those up on there and I wouldn’t get anything according to that. She put I need help with going to the toilet which I 100% don’t need and it’s the only thing I personally wouldn’t give myself marks on.

    The dots/ticks should be against all the activities with A being zero.

    So the assessment report was not as expected wrong information recorded. Don’t wait for the decision letter but start putting together your reconsideration appeal. You can work your way through the report correcting the wrong information and supplying supporting information and evidence as to why the HP has miss awarded you on the questions you are requesting a reconsideration. For example. Question 1 I am unable to prepare food using fresh ingredients as I can’t stand for long and have poor grip . There is no room for a chair or stool as I would not be able to move safely from the stove to sink and back. The HP awarded me 1 point when it should had been repeat through the report being honest with your answers and provable evidence submitted. Having a copy of what you sent in your application form is helpful as you can reference to that .

    A few things not quite right here. The HCP doesn't award points, they make recommendations only. Focusing purely on the assessment report isn't going to get anyone a PIP award.
    For preparing food, the lowest points possible is 2, for using an aid. Carrying things around the kitchen is not considered in any of the descriptors.

    Nor would the particular spatial arrangements in a claimant’s home be taken into account when assessing PIP. So whether or not a chair or stool would be required is relevant but the space available for a chair or stool is not relevant.

    It looks as though I’m getting lower on both. I’ll take it but I’m definitely going to do the MR. I haven’t had the letter yet but I phoned up and the automated system told me what I’ll be getting money wise. Anything will help honestly but I want to make sure they know the truth about my condition. My assessment was only 3 weeks ago so it was definitely quicker than I expected.

    You shouldn’t rely on the automated payment line because it has been known to be wrong. You need to wait for the decision.
