Creating a Journal for PIP supporting evidence

Hi everyone, hope you’re all having a great weekend.
I’m sorry for being a nuisance, I need to provide supporting evidence with my pip claim.
I was told to create a daily journal (how my mental illness effects me in a day to day basis - depression and anxiety)
I feel really overwhelmed right now, I’m not even sure how to start or go about it, are there any help online, a template I can use perhaps ? Any websites I can go to and use , a template would be helpful to be honest.
