You can challenge that decision by requesting the Mandatory Reconsideration (MR) within 1 month of the date of the decision. You should put your request in writing stating where and why you think you should have scored those points. Include a couple of real world incidents of exactly what happened the last time you attempted each descriptor that applies to you.Adding detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were.Timescales for MR decisions are 2-12 weeks. The most likely outcome will be the decision doesn't change. If this happens you can proceed to Tribunal.2

Am so confused x0

It’s probably not a lie so much as human error made under pressure of being overworked. It’s possibly gone down as a video call because it was scheduled as one and have failed to amend it.2

Because the descision has now been made all you can do is to do as Poppy Suggested and that is to put forward in writting a detailed Mandatory Reconsideration.I personally would also write a letter of complaint to the assesment provider and also maybe consider copying you MP into the letter (send the same letter to both Assessment provider and Your MP and put the addresses of both on your letter so they both know they have both been contacted with the same letter) Explain in detail the error that was made.Contact You MP https://www.turn2us.org.uk/Benefit-guides/Challenging-a-Personal-Independence-Payment-decisi/Tell-your-MPContact Details For Capita https://capita-pip.co.uk/en/contact-usContact Independent Assessment Services https://www.mypipassessment.co.uk/feedback/

Concentrate on the MR first and then once this is done and the letter is returned you can then make a complaint as advised above.
