OK, I receive contribution based esa topped up with income related esa (used to be on incapacity benefit) and also in the support group. Also receive pip. Because my contribution based esa is topped up with income related esa, does it mean I should be receiving the cost of living allowance? Hope the above makes sense.

If you receiving some income based ESA during the qualifying dates you should have received the CoL payments.
