Could it be an issue if my answers are different to what's on the form to what I say on the phone?

Hi guys and girls
Hope your all well!
Got a question about the assessment.
I filled out the application around 5 weeks ago and have my telephone assessment in about 10 days times. Within the 6 weeks my health has got worse. If my answers are abit different to what's on the application form to what I say on the phone, could this be an issue?
For example I've said I can walk 50 metres. Now I can barely walk 15metres. Should I tell the assessor I understand I've put 50 metres on my form however in the last 4-6 weeks I can only mange around 15 metres.
I don't want it to come back on me that I've contradicted myself.
Just a couple of things have changed since the application form would this go against me if basically what I'm wondering lol.
I want to be honest but don't want to be penalised for saying different information.

But if you make the Assessor aware before the assessment starts that you want to record the assessment, then the Assessor will read out the conditions (the recording won't be used for nefarious purposes) which allow you to record the assessment, then you can go ahead and record the assessment. But they must give permission to record, especially because you may need to refer to the recording in the future.
