Proof of disability

I need to provide a builders with a "proof of disability" in order that they can apply to the local council for free, planning permission for an extension.
What will be a suitable document ?
I have my PIP certificate of entitlement - but that goes in to more detail than I prefer to give to a builder....

    I don't have one yet
    I only transfered to pip last year!

    You should still have received the annual uprating letter.

    No I haven't - I only transferred 11 months ago !
    The only docs that I have from DWP is the PA3 (which goes into way more detail than I am happy to provide a builder with) and the Statement of Entitlement (which, looking at now - will hopefully suffice ?)

    Transferring 11 months ago makes no difference to receiving the uprating letter. You can try the certificate of entitlement but this is usually for vehicle tax (VED)

    Since this is only to obtain planning permission,free of charge - I only need to prove that I am disabled - not that I am receiving PIP - So any proof should be accepted ?
    There must be many disabled people who are not receiving PIP - But are entitled to many things such as free PP ?

    You could photocopy your award letter, and cross out using a thick pen anything that you wish them not to see.

    I'm not sure a PIP letter alone does prove disability legally?

    I think you'd need something like an occupational health assessment.

    If PIP letters are accepted, then maybe something like Blue Badge entitlement is also accepted?
