First time PIP form - any in person support

Dear Community,

I finally received the form but somehow it only has 10 days to submission deadline.
I am looking for support preferably in person.
Most of what i am going through is not really visible. and also even though i am fluent i often misunderstand forms like this or even emails until later. 
I communicate better in person. 

Any recommendation where I can ask for help?

I have FND, undiagnosed possible dysgraphia /dyspraxia extreme fatigue, sleep issues, and falling asleep at random times, bladder and bowel issues,  weakened left side including some muscle loss, recently tennis elbow as a result of overcompensating for left side, diabetes, and my mental health is very much affected by all thats been going on - Its like there are days I can do everything perfectly fine but then cant even get out of bed for 2-3 day in a row.

Any advise would be highly appreciated

PS- if awarded - is PIP on top of universal credit or does it come under the same allowence.?

    You can ring PIP to ask for extra time to return the form.

    If successfully awarded PIP is a separate payment to UC.
