What is the most recent dated / version for the downloadable PIP2 form please?

Am I right in thinking that the only downloadable PIP2 form is the form which is clearly marked "SAMPLE COPY" on all of its pages?
I would like to download this form, and it's called -
PIP2 claim form, how your disability affects you.
I wondered what the very latest version of this form is please?
I found a version of this form which is dated January 2021, and the link to this form can be found here on the turn2us website -
This form appears to be two things rolled in to one - the first 12 pages appear to be an information booklet section, and the next section appears to be the PIP2 form itself.
Thanks a lot for any help on this matter, it would be appreciated!

However they are from 2018 and so the combined 2021 form you have found is obviously more up to date.
The contents are basically the same anyway.1 -
Thanks. Yes I noticed that one too - that the govt form appears not to be the latest version. I wonder why?!

Good luck!There are no downloadable copies (other than samples) of any PIP form anywhere on the internet, this includes the gov.uk websites. Local Authorities will not have access to any PIP forms.A claimant will always receive the most up to date forms because when you start your claim you are sent a form in the post. Some people are offered to claim digitally and when doing this they are sent a link via email.1
