Care Summary Record

Hello everyone.

I have recently requested a print out of my Care Summary Record for a PIP claim. Before I requested it my brother/carer had to help me sort out my records because my local GP Surgery decided that my ongoing diagnosed problems needed amending. They decided to put some of them in the significant past section when they should have been left in the active and ongoing section. So after a few days and a visit to the Doctor we managed to produce a Care Summary Record that should be suitable for the PIP claim. My question is will it be ok just to use the first two pages showing my diagnosed problems and medication. We have twelve pages to this Care Summary Record and we are a bit concerned about whether the DWP will ask questions about the other ten pages. We have nothing to hide its just we don't want to add any insignificant information that will add bulk to the claim pack.

    @UrbanPigeon I think adding the first two pages and putting that you have more information on request would be great. They sometimes call if they need more information. So, to know you have that would be good :)
