Delayed DLA Renewal

My partner had a letter today to say her Carer's Allowance is ending in two weeks. After a bit of investigation we found an old letter that included our daughter's DLA end date as 10/01. I had a look online and there doesn't appear to be a renewal form online - I guess that's because each renewal is different. We plan to call DWP on Wednesday to see what we can do. We've not had a renewal pack but Royal Mail are not the most reliable and we often have to hand deliver our neighbour's post, sometimes a few streets away (similar sounding street name).
I was wondering what we should expect when phoning DWP? Is there any chance of getting a renewal form returned in time? Is it worth asking for an extension?
