Hi @Ada - as Albus has linked to you ring 0800 917 2222 to start a PIP claim. You answer a few questions as can be seen on the government's website: https://www.gov.uk/pip/how-to-claim & they'll send out a claim form. Perhaps, if you haven't already, you'll try this breathing exercise which I hope helps: https://www.torbayandsouthdevon.nhs.uk/uploads/reconnect2life-1-deep-breathing-exercise-handout.pdf I'm used to using this daily (& has previously helped with panic attacks), it also aids relaxation, & with practice can help in reducing pain. Practice when you're feeling OK to get the hang of it.

Hi @Ada - your partner can phone up, so long as you're there as they may just wish you to quickly confirm that you're happy for him to speak on your behalf. If that doesn't work for you as I can see it may not, you can write in to start your claim instead. From the govt's website:
You can start a claim by post instead, but it takes longer to get a decision.
Send a letter to ‘Personal Independence Payment New Claims’.
You’ll be sent a form asking for your personal information, such as your address and your age. Fill in and return the form.
You’ll then be sent a form which asks about your disability or condition.
