PIP Assessment Report - please advise

Hi all,
Hope everyone is as well as can be. I have received my health assessment report and wanted advice on whether I should contact DWP what I might think is an error.
Planning and following journeys - assessor said "therefore, it's likely he needs assistance to effectively plan and follow the route of a journey safely, repeatedly, up to standard, and in a timely manner on majority of days"
I agree with this statement as the assessor was very factful in their report. However they have said that and then marked box 11(a) which is cancelled plan and follow the route of a journey unaided.
There is a lot of copy and paste in the report which is usual but wanted advice should I raise this now so decision maker can be alerted prior to reviewing it.

To be clear is that what they wrote in the justification box underneath the descriptors in the report? If so then you can contact PIP to ask if a decision maker can contact you before making a decision. There's no guarantee they will but it's worth a try. Or you can tell them your thoughts over the phone and they will add some information to the system. Although they do mostly go with the report, it has been known for them to go against it.
Once a decision is made if you're not happy you can challenge that by requesting the MR.0 -

Thank you very much for your help. To confirm yes they marked box 11 (a) which is can plan and follow the route of a journey unaided.
Then in the justification box they stated why I would struggle and summarised by saying "therefore, it's likely he needs assistance to effectively plan and follow the route of a journey safely, repeatedly, up to standard, and in a timely manner on majority of days"
The box and justification clearly contradict and wanted to bring that to their attention. I'll do as you have advised and will ring them tomorrow and request some notes are put on for the decision maker and consider contacting me.
