Found text saying that I had a message from UC on my journal

I found a text on my phone about 2 to 2 and a half hours ago on my phone saying that I had a message on my journal from UC,so I logged into my account and found a message saying that they need to know more about my health condition or disability, but they didn't say which one they need to know more about.

They said that they'll be sending me the How your health condition or disability affects you form (UC50 I think it is called) in the post.

The only one I can think of wanting to know more about is my recent health condition which is the start of a catteract in my left eye, which doesn't affect my eye sight at the moment but it will in the future.

The diabetic optician who diagnosed me with the start of a catteract in my left eye said that he's not worried about it for the time being, so he's leaving my diabetic eye checks to yearly for now.

I also have yearly NHS eye appointments for my eyes at my local opticians too.

    Hello @BrianMcFaddenfan07, have you been submitting Fit Notes to UC? Or have you previously had a work capability assessment for LCW or LCWRA?

    A UC50 is sent for Work Capability Assessments or reviews.

    Hi @Adrian_Scope I was put on LCW back in 2019 after my dad passed away. I was told that it would only be for 3 years I think, but I was never sent a form or anything for a review, but when my advisor at my local job centre asked me about it I told that I was put in the LCW, but no-one had contacted me about a review since, so she said that she would contact them about it.

    I've not sent any fit notes to UC for years.

    Although I did find a couple of old ones belonging to either myself,my fiancé or both of us from when we were living in the North East of England. I'd put them in a folder with some other paperwork that we didn't want to lose.

    Have you reported a change in your health or your recent cataract diagnosis to UC? If not, it's possibly just a review of your LCW @BrianMcFaddenfan07.

    They have referred you for another work capability assessment. In a few days you will receive the UC50 form In the post, which you will need to fill in and return with all your supporting evidence. It will be the same form you filled in the last time.

    I don't remember filling the form in the first time Hun as it was not long after my dad lost his short but very bravely fought battle with terminal lung cancer. I was in pieces and couldn't think straight.
