Is an OT assessment enough evidence for PIP review??

I have my first ever PIP review after 5 years. The issue is my new GP won't provide a letter (my previous GP was amazing and did no charge for my initial PIP application) but I do have an OT assessment from last year November which outlines my medical condition, what I cannot do/how it affects me and also my need for a more suitable home but it's over a year old and even though my needs haven't changed will this be sufficient? I also have a medical summary from my GP but it only outlines my medical condition and current medication, with no mention of what I can't do due to my medical condition and disability. Oh I do have an appointment letter for pain management clinic too that I can include but the appointment isn't until after my review form is due to be returned, so I can't wait for that evidence
