Hi I've awarded full pip I have pulmonary fibrosis and severe emphysema. I am hoping for a lung transplant and heard I can live a full healthy life after it's done . Would this then effect my pip and my car I got on disability?

If your conditions improve in the future and this has been the case for at leat 3 months and expect that improvement to continue for at least 9 months then yes you would need to report the changes.
For the mobility scheme, yes you can join that because you have Enhanced mobility PIP. You would exchange the mobility payments for a vehicle of your choice. Do be aware though that some cars require you to pay an advanced payment before you collect the vehicle. Full details here.
