Tips on applying for DLA

My daughter is 5 and has moderate hearing loss. She's waiting on ENT appointment to eventually get grommets which may help but we've been told it'll be at least 18 months before the initial appointment and then could be another year for the surgery.
Do I add that info into the form? Because the grommets could help?
It's not just hearing we are applying for. My daughter is also autistic, has speech delay, and diagnosed anxiety. She doesn't sleep through the night (Although I am unsure whether that is considered at her age?). This was last night which is pretty typical of her sleep:
Bedtime at 7, awake at 9 for about 30 minutes.
Awake again at 11, took an hour to calm and settle back in bed.
Awake at 1.15, took until 1.45 to sleep.
Awake at 3.20 at which point I just slept in her bed.
Then she got up just after 5 and wouldn't go back to bed.
She's been prescribed melatonin but it makes no difference.
Any tips for filling in the form? I'm overwhelmed by it

Hi @Sammy298 - & welcome to the forum. You need to show that your daughter has substantially more needs than a child her age would have who didn't have any problems.
