You can challenge that decision by requesting the MR within 1 month of the date of the decision. If you do request it later than this you will need to tell them why you didn’t request it within the first month. You should put it in writing stating where and why you think you should have scored those points.
Most MR decisions remain the same, if this happens you can proceed to Tribunal.

@dieselsmum Hi yes I understand your pain it really painful, unfortunately my body reacts to a lot of medication so nothing working for me, I'm suffering with all these issues. I got my Pip result this morning they more or let score me almost zero on everything, plus they put things in the report that I didn't say, so not sure what I'm going do now because I am suffer with chronic pain and Fatigue.
I hope things work out for you.0 -
@poppy123456 Thanks for your advice, I am left feel even more depressed by this PIP thing don't think I can cope with a appeal because I already suffer with depression and anxiety and now this as made it 100 times worse.0

When they write the report it’s based on their opinion of everything you said in your form, any extra evidence you sent and what you said during your assessment.I wouldn’t expect it to say exactly what you said, if it was there would be no point in having the assessment. You have plenty of time to think about what you want to do because you have 13 months. If you request the MR after the first month then please do forgot to state why you didn’t request within that time.1

@poppy123456 Hi yes I know it may not be word for word, but there are things written that I didn't say all.
Thanks for your help.0 -
You're missing the point i'm trying to make. It's not about writing down what you said. It's still their opinion based on everything i mentioned above.I realise it's upsetting but you need to move on from that if you're going to challenge the decision. It's fine to mention a couple of obvious errors or contradictions but then put the report to one side. Then concentrate on where and why you think you should have scored those points.1

@poppy123456 Hi yes thanks for your advice, I'm just so depressed with everything0

Poppy is correct focusing on what was in the report will not help you get an award. You need to tell them why you should get the points in each applicable descriptor. Remember they don't just use the information at assessment, they rely on information you wrote in the PIP application..so many people go to appeal and end up focusing on "lies" in the report and end up no better off.0

I can hear how you're so depressed with everything @Palmtree125. It sounds really difficult for you at the moment
You have done so well to share how you are feeling with us. Would anything, in particular, help you to feel less depressed at the moment?
We are here for you if we can do anything to support you, even if that is just listening to you!0
