How can I access an advocate to support a young man with crohns attend a pip appeal? His condition is not under control and he struggles to communicate his needs.

This link will tell you what advice agency is local to him. https://advicelocal.uk/welfare-benefits you will need to look for something like a law centre or Welfare Rights. Citizens advice no longer represent at Tribunals. If there's nothing near to him then does he have a family member or a friend that can support him during the hearing? Even with representation, he will still need to speak for himself. Lots of people attend Tribunal hearings without representation.
