How do I get my LCWRA letter?

How do I get a copy of my limited capabilities to work with work related activities that letter was issued 12 April 2021 and I have a assessment on the phone with doctor date 
 7 of. Apni 2021. I’m also on sick note starting from
18. March 2020 till
continue  without any day gap. But I’m not getting any of the rest of the benefits entitlement benefits from universal credit and I’m never contacting by my case manager with in past 3 years.  I’m going more sick and depressed because of uc  unprofessional and unlawful actions il   I hand over my assessments decisions letter by my hand personally in to Rochdale job centre person name

Do you have access to your UC journal? If so, a letter should be on there.

Do you mean you've not been recieving the LCWRA element since being awarded in 2021?
