If the form arrived addressed to your mother then that suggests she’s his appointee so she will need to sign the form.I would appreciate it if members wouldn't tag me please. I have all notifcations turned off and wouldn't want a member thinking i'm being rude by not replying. If i see a question that i know the answer to i will try my best to help.

Sorry another quick question, do I fill it out as if I’m my son or on his behalf, for example do I put I can’t cross the road safely, or he can’t cross the road safely
many thanks -
As my daughters appointee i will always fill in those forms in as if i'm her. So for example, if he's unable to cross the road safely you write, "i'm unable to cross the road safely because"... you need to make sure you give a couple of real world examples for each activity that applies to him. Include detailed information such as where he was, did anyone see it, what exactly happened and what the consequences were.I always use extra sheets of paper because there's never enough of room on the forms to give all the information. Make sure you include his name and NI number on everything you send.I would appreciate it if members wouldn't tag me please. If found to have LCWRA he'll be entitled to an extra £390.06/month from the 4th month after he reported his health condition. You can see the descriptors here for this. https://www.benefitsandwork.co.uk/universal-credit-uc/uc-faq/limited-capability-for-work-related-activity
