Hello,
Is there a way to complete the PIP 1021 Change of Circumstances form and print it out from my computer? I am 66 and my handwriting is becoming illegible.
I am not sure if to try and write in the answers and also include a detailed typed response to the questions.
I would be grateful for any assistance, please.

Type your answers and print out on separate pieces of paper. Include your name and NI on the top of each page. On the form just write 'see attached' on each part. Send back the form, the extra pages and a copy of any extra information you want to send.0
