PiP back pain evidence advice

Hi all , im considering applying for pip as i have issues with back pain for the last few years, ive seen a few physios but im mainly on pain killers as it is mainly muscle/nerve pain, walking is painful bending etc i have some good days and some real bad days im just worried that the only paper work evidence i have really is the request for painkillers from gp and the few physio appointments i had. i see some people ask family or friends do do a letter? i could ask my mum as she helps me clean as it can  be a difficult task for me at times, any advice on this would be great , and also do they mainly do over the phone assemsents now ( would be much easier for me then have to drugged up on painkillers to manage the walk), thanks in advice for any help

    PIP is not based on a diagnosis but the problems you have completing the 12 PIP activities that apply to your reliably. The best evidence you can give is your own personal written anacdotal evidence about the reasons why you cannot complete the PIP activity reliably (safely, repeatedly, in a timely manor and to an acceptable standard) To increase your chances of getting an award you need to give detailed written personal anacdotal evidence on your PIP2 Questionnaire form. If there is not enough room the type or write out on seperate sheets and include those but you will need to reference those sheets on each activity on your PIP2 form.
    If you have not started your claim yet then I advise doing research on PIP and writting out all of your detailed written personal anacdotal evidence and gather any medical evidence (summary care record from GP? supporting letters from family members? disability diary?) then when you are happy with your claim telephone PIP to start the claim process.
