We can't actually help you with the form itself but we can advise on what you should do. Is it a PIP review form or a PIP2 form?Either way, you should include as much information as possible without telling your life story. Do not leave anything out, even if you think it isn't relevant because it maybe. The most important thing to concentrate on is real world examples of exactly what happened the last time you attempted each descriptor that applies to you. You sould include at least 2 for each descriptor that applies.Adding detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were. You should aim for at least half an A4 side of paper per descriptor that applies. There's also a guide here for the PIP2. https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/pip/help-with-your-claim/fill-in-form-pip/A guide here for the review form (AR1) https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/pip/pip-changes-and-reviews/help-with-your-pip-review-form/Any extra evidence you have to support your claim should be sent with the form. They very rarely contact anyone so do rely on that.If you need extra time to return it you can ring to ask and they will give an extra couple of weeks.
