Disabled eligibility to VAT free services

I have to fill in a DISABLED DECLARATION form for VAT free building work
What do I put in the "that I have the following disability or chronic sickness" section ?
How much detail do I need to provide ?
Will "permanent mobility issues caused by knee and back problems" suffice ?

    Qualifying for VAT-free building work

    You’ll only be able to have eligible building work VAT-free if you’re chronically sick or disabled and the building work’s being done in your own home. You do not need to be registered disabled or eligible for any other benefit to qualify for VAT-free building work but you must be chronically sick or disabled.

    What HMRC mean by ‘chronically sick or disabled’

    For VAT purposes, you’re chronically sick or disabled if you have a:

    • physical or mental impairment which has a long term and substantial adverse effect upon your ability to carry out everyday activities
    • condition which the medical profession treats as a chronic sickness (that’s a longterm health condition)

    @wibbles I always hate that question on any form. I usually write "diagnosed with..." and then put a note that says please contact if more information is needed. I would say as your wife helps care for you to put a sentence to say to contact her if by phone in that box too. Just in case they miss it somewhere else on the form.

    nb Only extremely limited building work is eligible for VAT free work - NOT kitchens (even though I cannot even enter our kitchen in my wheelchair (and cannot exit the rear house door for space reasons) - certainly cannot cook or access the kettle or anything else...
    We are currently 1/3 way through building a VAT free wet-room / bathroom / toilet which is absolutely necessary for health reasons !
    The rules are outdated and need major modification
