Your thoughts - wheelchairs and home adaptations

Hello community!
My name is Finbar and I work for a charity called Access Social Care. We provide free legal advice and support to people in England to get the care they are entitled to.
We've developed a legal chatbot that delivers legal information about community care and welfare benefits, to older or disabled people, their families, and carers. The chatbot is designed to take the user through their situation and identify a letter that they can send to help resolve the problem they face. The chatbot is free to use.
We're currently in the process of adding new content to the chatbot in relation to securing equipment, especially around wheelchairs and adaptations. We'd really appreciate your thoughts and experiences in this area around any of the following themes.

Wheelchairs:
- Can you tell us how you went about getting the wheelchair?
- How long did it take to get the wheelchair? What affected the length of time it took?
- Is it the right wheelchair for you? If not, what were the blockers to getting the right wheelchair?
- Did you go anywhere for advice when you were going through this process? Did anything in particular help?
- How was the wheelchair funded?
Home or equipment adaptations:
- Can you tell us how you went about getting this equipment or adaptation in place?
- What went well, what didn’t go so well?
- Did you have to have an OT visit as part of that process? If so, how was that arranged?
- How long did it take to get the OT assessment?
- Who funded the eventual adaptation or equipment?
- Did you get any advice during this process? What was the most helpful advice?
Disabled Facilities Grants:
Can you tell us how you went about getting the Disabled Facilities Grant?
What went well, what didn’t go so well?
