Pip face to face assessment

Hi I'm new here, I had a face to face assessment and 24 minutes after the assessment finished I got a text from PIP saying saying the health professional has sent us their report, this seems very quick, any way a few weeks later I got a letter awarding me zero points, i have asked for a MR, and I also asked for the PA4 to be sent to me, on the PA4 it says evidence used as PA3 dated 04/02/2020 this cannot be mine which I have pointed out in my letter to them, because I was awarded PIP on 06/01/2020 a month before this PA3 that they have used was created, it also says for every descriptor that it was previously advised unaided, ? I am confused to why it would say this, when I have been awarded DVL/PIP since 2009, yet the report suggests that I have never been awarded anything, I rang PIP again about this and was just told that the case manger would look at everything, I was also advised to report the assessor which I have not done yet, am I right in thinking that the assessor has somehow used somebody else's PA3, to come to they decision of zero points, also a lot of the information on the PA4 is not correct, any thoughts on this would be very much apricated, I'm sick with worry thank you
