I messed up on the on the paper assesment

I did not put half the things and most importent things on the paper work and Questionair they send you, Is there any way to add these things now or is it to late, things such as how my condetions mainly mental have impacts on things, will they ask me these qs in the telephone call or only ask me about the stuff i put on form ??

You can send more information at anytime but I see you have your assessment on 1st May so I'd advise you to send it to the health assessment provider, rather than to DWP. Please make sure you put your name and NI number on everything you send.
The most important part of a PIP claim is the form you send. The more relevant information you give, the better it will be for you.
I can't tell you what questions they will ask because I don't know how your conditions affect you.1 -

That the address for PIP. You need the address for the health assessment providers, which ever one is dealing with that, whether it’s capita or Independent Assessment Services. There will be a telephone number on the appointment letter you received, ring them and ask for the address.0

I never got an letter with appointment I had a miss call rang it back and they said booking team had made me appotiment for 1st of may.0
