UC50, mental health

First, new here and I wish this type of support had been available a long time ago.
I'm filling in my UC50 form and have a couple of queries.
My mental health condition varies and I know must apply for >50% of the time. Basically, under some scenarios I get worse so avoid the pressures in the first place as much as possible as I don't want to spiral down. I therefore may not display their descriptors 50% of the time even though the condition is always there. Is this enough or is it a Catch 22 situation? (I think I've explained this correctly).
Is it worth putting a family member who knows the condition well on the form, are they likely to be contacted?
Thankyou.

Also, are they after a detail from a certain time period, eg, the last month, three months, a year or more?0

It’s the majority of days that count so at least 50% of those days. There’s a guide here that may help. https://www.citizensadvice.org.uk/benefits/sick-or-disabled-people-and-carers/employment-and-support-allowance/help-with-your-esa-claim/fill-in-form-esa/It says ESA50 but the UC50 is identical.
Have you also taken a look at the descriptors? If not then you can see them here for LCWRA https://www.benefitsandwork.co.uk/universal-credit-uc/uc-faq/limited-capability-for-work-related-activity
LCW is here https://www.benefitsandwork.co.uk/universal-credit-uc/uc-faq/limited-capability-for-work
As for contacting anyone, they very rarely do this. If they do they may send a UC113 form to your GP but you shouldn’t rely on them doing that.1
