Hi @Alex24, welcome to the community. What kind of help do you need with the form? I'm sure our lovely members will be able to offer some advice if you've got any questions. I'll put your post into our DLA section where they can find it more easily.
If you're struggling, some people find it helpful to speak to Citizens Advice or a local benefits advisor, so that could be an option too0 -
Putting my post on to the DLA section would be very helpful - thank you0

Hi @Alex24, I'm looking at a sample claim form online and 37 is asking if your child has any educational health care plans in place. Are you using the same form?
If some of the questions don't apply to your child, that's okay. Some questions will be more relevant than others, depending on why you're claiming. It's worth taking some time to consider each question, as you might find that certain information could support your responses to the more relevant questions.
Contact has some tips on filling out the DLA form which might be of interest too.
