UC50 form. Where can I go to get help filling it?

Hey all.
So my UC Journal claims I'll be getting a WCA soon and a UC 50 has been sent.
I just downloaded a UC50 form to see what I'm in for when it comes and I understand it and I don't. I get very overloaded quick and my mind can't hold on to info for long due to it.
One example is, I could be typing this, and forget completely what I've typed prior and then forget the next part or forget completely why I'm writing this.
I also have to block a part of a sentence or I can't read it because my mind bounces from one part down the page to the top. It's hard to explain.
Can you all tell me services that could help me write it and help me keep track? It would great if they could help me on the phone if possible? Sorry to ask. I might not need the help but just incase. Thanks
