Pip mr
Hi, I applied for pip got 6 points, i have called for a mr but will follow this up with a letter and more evidence.
On activity 1 - preparing food i scored 2 on descriptor d. Needs prompting. I stated on my form and to the hp that i do not prepare or cook my meals and that my husband does due to my ocd.
Activity 6. Dressing & undressing. The hp wouldn't let me explain how i needed help, she cut me off and said it's just how long it takes to dress? Then proceeded to put on her notes that i do not have any rituals & need no help so scored 0
Activity 11. Planning & following journeys i scored 0. Even though i told the hp i can not follow a familiar journey without my husband I went into detail about what happens if he is not with me etc. Their reasons for scoring 0 was because I used to drive myself to work so that means i can complete a journey unaided. I haven't driven myself to work in over a year, on maternity leave but was signed off sick early on in pregnancy and during this time my mh got really bad and won't be able to go back due to mh.
My husband will be writing a letter explaining how he cares for me & what happens if he doesn't. I have also asked my therapist for a supporting letter. Could I also use my sick note as proof to show that I have not driven to work in over a year ?
And how much detail should I provide when writing an MR, I have seen loads of different answers as to how much detail and don't know how to approach it.

Ideally, what you should have done is request it through a written letter, explaining where and why you think you should have scored those points, rather than ring to request it. Now, you've rang, they do not have to wait for a letter and/or any extra evidence.
What you need to do is give a couple of real world examples of exactly what happened the last time you attempted each descriptor that applies to you. Include detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were. You should aim for at least half and A4 side of paper per descriptor that applies to you.
Try not to concentrate too much on the decision letter or assessment report. (if you have a copy) It's fine to point out a couple of obvious errors but then put them to one side and forget about them.
The most likely outcome of the MR will be the decision remains the same. If this happens you can proceed to Tribunal, where either appearing in person or having a telephone/video hearing will give you the best chance of a decision in your favour.
A fit note isn't evidence that you qualify for PIP.

Ok, thank you very much. I realised I should of just sent a letter after the fact, When i called i got caught off gaurd didn't know they ask the reasons why you disgaree thought it was just a notice to let them know you wanted an mr. I did let them know I would be sending extra evidence
Even though it is not evidence itself, could i use it to disprove their point that I have not driven to work because it has the date on it?
