LCWRA ending soon

Hi all ,
I have been awarded lcwra for 17 months ending by this 22 August, yesterday I received a call from job centre asking me how do I fit for work , I explained that my condition still the same and I'll have a surgery beginning of September, he asked me to list this on health condition and bring a fit note from gp !!!
May I ask how do I renew my lcwra , do I fill up a new form and ask them to extend the current one , appreciate any help

This doesn't seem quite right to me. If it is UC you are claiming then they should send you a journal message they are going to reassess you. Then a form goes in the post. If it is ESA then a reassessment form comes through the post.

I'm going to ask through journal about reassessment form , Yes we are claiming UC ,They booked me a phone appointment end of July without offering any reassessment thats why I Was confused and to be honest I Don't know about renewal.

LCWRA is an ongoing award until a new decision says otherwise, it has no end date.
You mentioned “we” if the appointment is for the person that has LCWRA then this isn’t correct. You have no commitments so shouldn’t need to attend any appointments. You can’t be sanctioned either. I would advise you to put a message on your journal and remind them that you have LCWRA and do not need to attend appointments.

Thanks poppy , may I ask then why it has an expiry date at 22 of August and It has been allowed for 17 months , should the renewal happens automatically or after reassess the health condition.
In the commitment, the job centre send me to accept the new commitment of work search otherwise my payment will be affected, and in the phone call he asks me for a sick note !
He didn't mention anything about reassessment or renewal, that's why I thought of asking here .

This was the letter from UC following my LCWRA decision it only mentions a review and reassessment there’s no end date I would assume it would be the same for all claimants with LCWRA or do they make up their own rules for a particular area?

That expiry date does exist. What it would be is your referral date but that doesn't mean you'll be referred on that date because it's just a guide. In fact, WCA for those that have LCWRA are still suspended. It's only those that have LCW that are having reviews at the moment.
The work coach who sent you the new commitments is wrong because you have no work commitments. Please put a message onto your journal and remind them that you have LCWRA, which has no end date. If you have no joy then put in a complaint.
It's the same for everyone with LCWRA and LCW, there's no end date.

I have attached a copy from the letter I received, perhaps there is something I missed it as I am new in this , the only thing I added in my condition that I have been scheduled for surgery beginning of September and my other medical conditions are the same , and after this I find a message of work commitments search and I have to accept my commitments,
I'll definitely write a message on my journal ,thanks for your answers and support:)

In my comment above, there's a typo, it should read "the expiry date doesn't exist" rather than it does exist.
In your first screenshot at the top it says for period 26/08/22 to 08/12/2022 but I don't understand that because it makes no sense. I also see that the letter was dated 17th January 2023, which is after that date.
Regardless, they are incorrect to make you attend appointments.
