How do I attach evidence pages to PIP claim form?

I have about 15 pages of evidence to attach, I tried to staple them to the form but the stapler won't go through that many pages. What should I do?

You don't need to staple them to anything. The staples are removed when it's added to the system anyway. Just make sure you include your name and NI number on everything. Make sure you keep a copy of everything for yourself.

OK I actually stapled them all before reading your post. Should I pull the staples out again? Should the pages just be loose in the envelope?

I always send supplementary pages, as Poppy said put your full name and NI number on every page. Then what I also so is number every page and mark it with what page it is out of how many so "page 1 of 10" "page 2 of 10" etc, that helps to ensure no pages are missed by the assessor. What I also tend to do is make it very clear in the PIP form that I sending extra pages so if I've addressed a certain question I note on the PIP form where inthe extra pages the notes are so in the box on the form I will write "see also supplementary pages, page 3 - etc.

I do the same as above, put name and NI number on them all, and I sign them all as well
