A letter from a GP isn't the best evidence to send because they don't know exactly how your conditions affect you. They don't spend anytime with you to know how you prepare food, wash, dress, undress, etc etc. If they know it's because you've told them and then it becomes hearsay.

Your real life examples of exactly what happened the last time you attempted each descriptor that applies to you is much better. Include detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were. Please also bear in mind that they will only take into consideration how your conditions were at the time the assessment took place. Any worsening of condition since then will not be considered.

As I advised in my previous comment letters from a GP are not the best evidence to send. This includes when you request a paper based review. A paper based review will only be completed if they have enough of information to do that. They are also quite rare.

Instead of the letter focus on the real world examples I mentioned above.
