I can’t remember exactly what it says because it’s a long time since I claimed UC on behalf of my daughter.

It should ask something like do you have health conditions, which you would tick yes. You do not have to list all your conditions because that would be done when you fill out the UC50 form.

Fit notes are formally known as sick notes and yes they are needed. What don’t you know what to do? I don’t know how to explain it anymore than I have.
