Confused about free options check up

Hi I'm jay. I have a optians appointment next week I'm on income related ESA and not sure if I need to fill out a HC1 form before I go I'm confused and don't understand it. Thankyou.

They will just ask if you are on any benefits so just tell them esa. I've never had to take in any proof. You will be entitled to a voucher towards any glasses you may need. I'm not sure how much it is now but they will tell you
