More information is needed. What is the Tribunal for?

Disability discrimination. I've been on reasonable adjustments for 4 years now. But my employer is now saying that they can no longer accommodate me.
3 months less one day
They start with Flexible working request .but it was only to get me 2 shift pattern
Using 3
Occupational Health has away to apply 2 shift pattern. But appointment letter was for a up-to-date medical report
Health and well-being meeting to try and impose 2 shift pattern . Nothing about my health and well-being. I was given 3 opinions.
1. Go against Occupation reports that state I could only work day shifts.
2 find someone to do my afternoon's job share. But again that was going against OH advice. I need to keep moving around giving my body chance to rest. Highly repetitive tasks. Covering holidays and absences
3 go part-time and lose 13,000 a year which I can't afford.
