Hi,
The form gets filled in online. It's not very difficult, just go through each question one at a time. It's nothing that you won't have been asked before in order to claim ESA. Just asks about your living situation, working situation, any children and any savings you may have.
You get two weeks run on of ESA from the date you submit the online form, then a gap of about 3 weeks before the first Universal Credit payment arrives.
If you can't cover those 3 weeks using your ESA, you can ask for an advance payment on your Universal Credit, but that does have to be paid back from future UC payments.
If you're in the Support Group for ESA, it is important to mention this in your UC journal as soon as you submit the claim. And do not provide a fit note if they ask for one. When ESA send your info across to UC, you will be put into the LCWRA group without the need for another assessment.

Hi @lulu28, welcome to the Scope online community 🙂 I know it can be worrying about what can happen. I hope OverlyAnxious' answer has helped a little.
If it also helps, we have an advice section on Moving to Universal Credit: Moving to Universal Credit and transitional protection | Disability charity Scope UK

That letter you received was just giving advice that support will be there for you when you do receive the migration letter, which can be anytime up until about December 2025.

Weren't you claiming Tax credits as well? It will only have the HMRC logo on it if you were claiming TC. If not then it will have DWP/UC on it. It's DWP that send out migration letters.
