Pip review form

is someone else allowed to fill in my pip review form for me and if so do they need to sign it in their name and should I sign it also? Should I send it back by recorded tracked delivery?

Yes, someone else can fill it in on your behalf. No, they do not need to sign it, you will need to do that. The only time someone else should sign your form is if you have an appointee.
All you need is proof of posting, which is free from any post office. Make sure you keep copies of everything you send.

Hi @KatieG - it's absolutely fine for someone else to fill in your PIP review form. If there's a reason for this, then it's perhaps worth mentioning. For example my son filled in my initial claim form as I have difficulty handwriting. We mentioned this at the end of my claim form as it was due to an old injury I'd had to my right arm, hand & wrist which I'd already mentioned in the form. I signed it, as you should do.
I don't know if it helps, but you could type your replies out, so long as you put your name & National Insurance number on any extra pages you use (& the question to which it relates). I didn't know that at the time, but would have been something which would have been easier for me.
Please with your review form give as much detail as you would have done with your initial claim form. Don't just say 'no change' for example without saying exactly what are the difficulties you still experience.
Try & give a couple of recent, detailed examples as to the difficulty you face for each applicable descriptor, i.e. when did it happen, where, what happened, did anyone see this, & were there any consequences to attempting/doing an activity?
Just get a free 'Certificate of Posting' from your Post Office. That will show exactly when you posted it, which is all that's needed.

Hey @KatieG, as Poppy and Chia have said, someone else can indeed fill in the review form on your behalf but you will need to sign it. It can be good to say why someone has had to help fill in your form for you. Ensure that the person filling in the form knows that everything has to be noted down like it was a new claim for PIP.

I use “she” when filling out my daughters form.
