Presumably that came from the DWP.

Yes I have asked the DWP to provide me with the assessment report without the name of the assessor redacted but I have not received a reply

I meant asking the outside agency that did your assessment, the agency that the assessor works for. Unless you live in one of the very few areas that the DWP themselves handle the entire process.

The 28-day deadline generally refers to when the form should be received by the UC Department, not when it’s sent.
