Question Regarding PIP Review Form

Hello,
So, my PIP review form arrived yesterday, and I just have a question(s) about the layout of the form.
It still has all of the activities, but it has 3 sections to each element, which says something along the lines of:
“Has your condition changed, and when did this happen?”
“What appliances/aids do you use to help manage your needs”
“Do you need any more help or what help you get from another person”
So, if your condition changed you obviously say yes, and give a rough estimate as to when it changed right?
Also, in the next part you would say any aids/appicianws that you use, and how they help you? (Could you also give examples in that part?)
And, my mam helps me a lot with several things, so could I include in that and say how my mam helps me in the next part where it says help you get from another person?
Any advice would be greatly appreciated. I applied for PIP in 2021, and I was awarded PIP in 2022, after winning at the tribunal.
Any advice regarding the review form, would be greatly appreciated. Thanks.

That is the PIP review form. Please don't be fooled by the shorter form. You still need to treat it as a new claim, even if there's been no changes to your health conditions. Please do not just write no change and nothing else.
You need to give as much relevant information as possible. Also include a couple of real world examples of exactly what happened the last time you attempted each descriptor that applies to you. Adding detailed information such as where you were, what exactly happened, did anyone see it and what the consequences were.
There's also a guide here that may help you.
When I fill out those forms I always find there's never enough of room to include everything you need to tell them so I use extra sheets of paper. Please make sure you include your name and NI number on everything you send. Also keep copies for yourself and never send originals.
If you need extra time to return it you can ring and ask and they'll give an extra 2 weeks.
As your current award was from a Tribunal it's even more important to fill out the form with as much relevant information as possible. If you have a copy of the Tribunal decision, then I'd advise you to also include that as part of your evidence.
There's extremely long delays with review decisions and you could be waiting up to about a year for a decision. Your current award will continue to be paid until that decision is made.

Thank you for sharing that information. I was going to use extra paper, because it’s impossible to write everything pn the 3 lines on the form they provide.
I am also going to explain the best I can, and also include examples etc..
Unfortunately, I don’t have the decision letter from the Tribunal. Is there a way to request that again? Or, can you still sign into to the website and access it, because when I was awarded it I could view the decision online via the tribunals website.

Also, I did take a photo of the decision letter. If I can find that photo, how would I include that in the review form? If I can’t find the photo, is there a way I can request that letter again?

There's no way of getting a copy of the letter from HMCTS and your online tracker is unlikely to show any information now. If you can find that picture then yes print that off and send it with the form.
Other than that the only other way is to make a subject access request (SAR) to DWP for the information they hold on you. You can request this between 2 dates, if you remember the date of that hearing and then make sure you include that date with the SAR. You can request this online.

Thank you for sharing that. If I can’t find the photo, then I don’t think I will request the letter, as that will most likely take quite a while. What do you think I should do?
Also, regarding the 3 questions to each activity do you think I should just say how my conditions affect, and if I use aids/appliances and who helps me? Rather than answer there 3 questions individually?

It's entirely up to you what you do about the Tribunal decision letter. Remember, you can send extra evidence at anytime once the form has been returned. The majority of people are waiting an extremely long time for their review decision anyway. I recently had a paper based decision, which took almost a year after receiving the PIP review form.
What I do when filling out the form is write a small amount on the form and then write.. "please see additional sheets of paper for more information." I then type everything and print it off. With the real world examples I usually aim for at least half an A4 side of paper per descriptor that applies to me. I've had 3 paper based decisions from that.
I'd advise you to do a little at a time and then take a break. It's really stressful filling in those forms and I always think it's very easy to miss things out if you rush them. I always ring to ask for extra time to return them too.
