I'm moving from Birmingham to telford I'm on disability and universal credit can you tell me if this will change or does it stay the same. Also do I have to make a claim for council tax

It will stay the same, after you move you'll need to let UC know your new contact details, and rent amount for UC (if applicable).
Yes you'll need to make a new claim for help with Council Tax via your new local authority. Do be aware that all LA's have different rules for Council Tax Reduction, so you might need to pay a different amount.
