Uc50 form sending more evidence

Hi all, my partner has been diagnosed with prostate cancer and we sent the uc50 of just before Christmas when we sent the forms of we were waiting on biopsy mri and bone scan results we now have that and my partner has unfortunately been diagnosed with prostate cancer he now waiting to receive radiation therapy and is currently on hormone therapy how do I now send evidence to them to let them know because as far as I’m aware if your going to receive radiation therapy you don’t have to go though an assessment I read on many websites about this has anyone got the email address I have to send it to our assessment centre is Maximus thanks
