Hi all

Just wondering if someone could help me with a pip form please?

Page 10 question 1c ....it asks name of treatment, therapy or operation?

Does this mean a list of medications I'm on as I stared this on previous page in the medication section?

Also for page 19 it asks for any therapies taken at home...does this mean medication or do I leave these blank if not applicable?

Hi,

Question 1C relates more to practical treatments. There's no need to list all of your medication again.

I think page 19 will be question 5C, managing treatments? If so, that's asking about any help you need for things like physio or dialysis. If you're only taking medication at home, that one probably doesn't apply in your case.
